Created for a customer who wanted to be able to send out surveys to attendees of a training courses to assess how effective the training had been. They wanted to pull their questions from a library which they could maintain and update. These questions needed to be flexible and shown to the user in a simple wizard format and allow images to be associated with the question to help the user. The customer also wanted to be able to report on who had answered a particular question and how they did.
