Bug fixes and cleaning up the template to make it more customizable without changing design by using configuration documents

Minor bug fixes and enhancements:

• Removed hardcoding of paths
• Added system configuration form/documents
• Changed environment variables to use config doc for prefix
• Changed Mail agent to lookup config for email sender
• Added "release implemented in" field on feature request/bug fix form, and changed release form to display all bug fixes and releases applied to that release
• Added status field to release form to make a release available to public or not
• Removed click target for Notes so document does not open as soon as you click on it, but requires double click
• Opening project related docs in new window to make it easier to navigate (ESC takes you back to the previous view instead of out of the app)
• Added Additional Editors field to some forms to allow master chef to give access to others
• Added discussion threading for project ideas/tasks/documentation/feature requests/bugs/releases/openings
• Changed "Discussion" to store discussions in the main PMT db itself rather than another discussion db, along with all the response hierarchy
• Changed the top nav to have dropdowns to jump directly to a project rather than having two clicks to go to a project
• Added a link to view all discussions in PMT
