Someone outthere may correct me, but unfortunately re: Page Title, you don't. What I do when I want to "refactor" a page is:
1. Create a new page with the new page title and copy the content from the old page in.
2. Return to the old page and look at the backlinks.
3. For every page that shows up in the backinks, edit that page and change all links to the new page location.
4. Delete the page with the obsolete title.

Yes, this is a pain. It's why I tend to slow down & think carefully when creating a new page.

The <code> question is easier. <code> is simply an HTML tag, similar to preformatted text (<pre>). Use like this:
<code>
Your monospace text here.
</code>

The exact appearance depends on your style sheet.

What <code> will NOT do is "syntax highlight" any code you stick in it (if that was what you were expecting).
