Description

Major overhaul of the template. Now includes subforms to use when you need some
basic serial approval routing on both client and web.

I stuck with some basic things here. I think it would be good to merge this
with what Jack Radcliff is doing with XML but I wanted this to be compatible
with IE and Netscape, etc. so things like the calendar picker are javascript
code.
